ABSTRACT

Objective To compare the diagnostic performance of PI-RADS v2.1 and PI-RADS v2 in the detection of clinically significant prostate cancer(csPCa) by Meta-analysis. Methods The major biomedical databases were searched (CNKI, CBM, Medline, and Embase) with the keywords "PIRADS v2.1" or "PI-RADS v2.1". The Quality Assessment of Diagnostic Accuracy Studies Tool v2 (QUADAS-2) was used to evaluate literature quality. Meta-analysis was performed using STATA17.0 and ReMan5.4 software. Forest plots were used to represent the sensitivity and specificity of PI-RADS v2.1 and PI-RADS v2 for each study. Sensitivity, specificity, positive likelihood ratio, negative likelihood ratio, and diagnostic odds ratio were combined, and diagnostic performance was evaluated using asummary receiver operating characteristic curve (SROC). Subgroup analysis was performed on three covariables: tumor location, threshold, and the nationality of authors. Results A total of 12 studies were included, involving 3 158 patients and 3 243 lesions. Forall zones and the whole gland, PI-RADS v2.1 had a larger area under the SROC curve (AUC) for csPCa performance, compared with PI-RADS v2. Subgroup analysis: PI-RADS v2.1 also had a larger area under the SROC (AUC) to detect transitional zone csPCa. Different diagnostic thresholds: when a score of 4 was used for the threshold, PI-RADS v2.1 had the maximum area under SROC (AUC) for csPCa performance detection. Author nationality: Researches of PI-RADS v2.1 in Chinese authors had the largest area under the SROC (AUC) in detecting csPCa performance. Conclusion Compared with PI-RADS v2, the diagnostic performance of PI-RADS v2.1 in detecting csPCa is not obviously improved and overall specificity is still low.

ABSTRACT

Objective:To evaluate the value of systematic education for primiparas undergoing epidural labor analgesia.Methods:A total of 240 primiparas who were suitable and voluntarily requested epidural labor analgesia, aged 20-35 yr, at 37-41 + 6 weeks gestation, with a singleton fetus, in vertex presentation without fetal abnormality, were selected.According to whether the primiparas and their accompanying family members had received systematic education on epidural labor analgesia during pregnancy, the primiparas were divided into 2 groups ( n=120 each): education group and control group.When the uterine contraction was regular and the diameter of the uterine orifice reached 2 cm or more, epidural labor analgesia was carried out.Before the begining of epidural puncture, the degree of anxiety of pregnant women was evaluated using the self-rating anxiety scale.The time of communication before analgesia, time of placing body position, and satisfaction of puerperae and their family members with labor analgesia were observed and recorded. Results:Compared with control group, the self-rating anxiety scale score was significantly decreased, the time of communication before analgesia and time of placing body position were shortened, and the rate of satisfaction of puerperae and their family members with labor analgesia was increased in education group ( P<0.05). Conclusions:Systematic education is helpful in relieving the anxiety of primiparas during epidural labor analgesia, increasing the efficiency of analgesia implementation and improving the delivery experience of puerperae.

ABSTRACT

In the past decade, relative proteomic quantification using isobaric labeling technology has developed into a key tool for comparing the expression of proteins in biological samples. Although its multiplexing capacity and flexibility make this a valuable technology for addressing various biological questions, its quantitative accuracy and precision still pose significant challenges to the reliability of its quantification results. Here, we give a detailed overview of the different kinds of isobaric mass tags and the advantages and disadvantages of the isobaric labeling method. We also discuss which precautions should be taken at each step of the isobaric labeling workflow, to obtain reliable quantification results in large-scale quantitative proteomics experiments. In the last section, we discuss the broad applications of the isobaric labeling technology in biological and clinical studies, with an emphasis on thermal proteome profiling and proteogenomics.

ABSTRACT

Objective To analyze the expression of miRNA-29a in U937 macrophages infected with Mycobacterium tuberculosis and its regulation of target genes.Methods The target genes of miRNA-29a were predicted with softwares miRnada,RNAhybrid and targetscan.The differentiation of U937 macrophages was induced by phorbol ester(PMA), the induced U937 cells were infected with bacilli calmette guerin (BCG).The expression levels of miRNA-29a and its target genes in U937 cells were detected with real-time fluorescence quantitative PCR(RT-fqPCR).The miRNA-29a over-and low-expression U937 macrophage cell lines were constructed and the levels of miRNA-29a and its garget genes were detected.The SPSS 18.0 software was used to analyze the data.Results As predicted by relevant softwares,the miRNA-29a regulate the expression of VEGFA,NFATC3 and TSC22D3 genes.After BCG infection,the expression of miRNA-29a increased to 1.33 fold(P <0.05), and the expression levels of VEGFA, NFATC3 and TSC22D3 were increased to 5.34,99.25 and 2.12 fold,respectively(P<0.01).In the miRNA-29a over-expressing U937 macrophages,the expression levels of VEGFA,NFATC3 and TSC22D3 were up-regulated to 1.35,1.29 and 3.38 fold,respectively(P<0.05 or <0.01).While in the U937 macrophages with miRNA-29a knock-down,the expression levels of VEGFA, NFATC3 and TSC22D3 were down-regulated to 0.07, 0.08 and 0.55 fold, respectively(P <0.01).Conclusion The results suggest that Mycobacterium tuberculosis infection can increase the expression of miRNA-29a in U937 macrophages,further targeting the regulation of VEGFA, NFATC3 and TSC22D3 gene expression, which may participate in the pathogenesis and development of tuberculosis.

ABSTRACT

Objective To investigate the effect of short course chemotherapy combined with broncho-vaxom on the prognosis of new smear positive pulmonary tuberculosis patients.Methods 80 patients with new smear positive pulmonary tuberculosis were selected as the research subjects.According to the random number table method,they were divided into observation group and control group,40 cases in each group.The control group was treated with 2HRZE/4HR chemotherapy,the observation group was treated with broncho-vaxom tablets based on the treatment of the control group.After 6 months of treatment,the sputum negative conversion rate,pulmonary lesions absorption,the incidence rate of adverse reaction and immune function were compared between the two groups.Results After 2,4,6 months of treatmentt,the sputum negative conversion rates of the observation group were 90.00%,95.00% and 97.50%,respectively,which were significantly higher than those of the control group(x2 =4.020,4.114,3.914,all P < 0.05).After 2,6 months of treatment,the total effective rates of the observation group were 87.50% and 97.50%,which were significantly higher than those of the control group (x2 =4.588,5.000,all P < 0.05).After 6 months of treatment,the IgA,IgG,IgM,CD3+,CD4+,CD4+/CD8+ in the observation group were (70.24 ± 6.19) %,(46.89 ± 6.25) %,(2.21 ± 0.39),(3.86 ± 1.43) g/L,(14.76 ± 2.58) g/L,(1.47 ± 0.65) g/L,respectively,which were significantly higher than those of the control group(t =-2.116,-2.575,-2.322,-2.138,-4.513,-2.599,all P < 0.05),the CD8+ was (18.85 ± 2.08) %,which was significantly lower than that in the control group (t =2.609,P < 0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups (x2 =0.251,P > 0.05).Conclusion Short course chemotherapy regimen combined with bronchovaxom can improve the new smear positive pulmonary tuberculosis patients with recent sputum negative conversion rate and lung lesions absorption effect,improve the immune function of patients and without increasing the incidence of adverse reactions,it is worthy of clinical attention.

ABSTRACT

Objective To explore the use of maintenance hemodialysis patients in the application of calcium dobesilate on serum Hs-CRP and TNF-α and renal function. Methods 86 patients with maintenance hemodialysis admitted from Xiangyang Central Hospital from October 2015 to November 2016 were divided into 43 patients according to the treatment group and 43 patients in each group. The control group Given conventional hemodialysis treatment, the study group on the basis of the control group given calcium benzenesulfonate treatment, and finally the results of the two groups of patients to statistics and summary. Results Before treatment, the difference of BuN, Scr, Hs-CRP and TNF-α between the study group and the control group was not statistically significant. The scores of BuN, Scr, Hs-CRP and TNF-α in the study group and the control group were significantly higher than those in the control group (P<0.05), and the difference was statistically significant (P<0.05). The serum Hs-CRP and TNF-α in the study group were significantly lower than those in the control group (P<0.05). Conclusion Calcium dobesilate can reduce BuN and Scr, improve renal function, reduce Hs-CRP, TNF-α and improve microinflammation. Patient side effects, improve survival, To improve the quality of life.

ABSTRACT

Objective To discuss the clinical significance of serum membrane attack complex (MAC)and complete factor H (CFH) level in lupus nephritis (LN).Methods Indirect enzyme-linked immunosorbent assay was used to detect MAC and CFH respectively in 30 LN patients and 25 healthy controls.At the same time other laboratory indexes were detected and the relation between the change of serum MAC and CFH levels and other indexes were analyzed.The data were analyzed by t test,Chi-square test and Spearman correlation analysis.Results The level of serum MAC in LN group was significantly higher than that in healthy controls and non-active group [(74±100)] vs (18±8) μg/ml,Z=2.777,P<0.05;(92±53) μg/ml vs (32±23) μg/ml,t=3.661,P<0.05].The level of serum CFH in the LN group was lower than that in healthy controls and non-active group [(311±146) ng/ml vs (412±76) ng/ml,Z=-3.139,P<0.05;(282±11) ng/ml vs (330±20) ng/ml,t=-8.333,P<0.05].The serum levels of MAC in the LN group showed negative correlation with the level of C3 (r=-0.603,P<0.05),and showed positive correlation with dsDNA,24 hours urine protein quantification,as well as the scores of SLEDAI and BILAG (rdsDNA=0.481,P<0.05;r24 hpro=0.694,P<0.05;rBILAG=0.709,P<0.05;rSLEDAI=0.613,P<0.05).The levels of serum CFH in LN group was positively correlated with the level of C3 (r=0.568,P<0.05),while negatively correlated with dsDNA,24 hours urine protein quantification,as well as the scores of SLEDAI and BILAG (rdsDNA=-0.443,P<0.05;r24hpro=-0.485,P<0.05,rBILAG=-0.639,P<0.05,rSLEDAI=-0.597,P<0.05).The pathology of lupus nephritis in the active group of 18 patients showed proliferation of glomerular cells and degenerated tubular epithelial cells,and those in the non-active group was mainly the proliferation of mesangial cells.Conclusion In summary,the increase of MAC and decrease CFH level accelerates the development of lupus nephritis,while they are positively correlated with the change of pathology in active lupus nephritis,and it would be good indirect serological markers for lupus nephritis.

ABSTRACT

Objective To investigate the effect of special CT monochromatic images on improving imaging quality in Budd-Chiari Syndrome.Methods 40 patients with Budd-Chiari Syndrome underwent CT venography (CTV)with gemstone spectral imaging (GSI).Using the optimal contrast to noise ratio (CNR)curve of GSI software,the optimal CNR monochromatic images of the infe-rior vena cava and hepatic vein were chosen.The CT value of the intrahepatic segment of inferior vena cava,hepatic vein and the same level liver tissue were measured,and the CT value of the same region were measured on 140 kVp mixed energy images and 70 keV images.CNR on the optimal monochromatic images,the 140 kVp mixed energy images and 70 keV images were calculated respectively,and the image quality of the optimal monochromatic image,the 140 kVp mixed energy images and 70 keV images were subjective scoring.One-way ANOVA was used for statistical analysis using SPSS1 7.0 software.Results The optimal keVs from monochromatic images were distributed at 40 keV (50%)and (5 1 ± 1 )keV (50%)for IVC and 40keV (25%)and (53 ± 3)keV (75%)for HV.The CNR and CT value and subjective score of IVC/HV got from the optimal monochromatic images were signifi-cantly higher than those from 140 kVp mixed energy images and 70 keV images respectively(P <0.01 ).Conclusion The optimal monochromatic images provide higher image quality of IVC and HV in patientswithBudd-Chiari syndrome.

ABSTRACT

This study was aimed to investigate the antitussive, expectorant and antiasthmatic effects of aqueous extracts and the chemical split fractions ofMori Cortex. Cough mice models induced by ammonia water were used to observe the effect on arresting cough. The phenol red expectoration method in mice was used to observe the effect on expelling phlegm. Histamine and acetylcholine mixture induced asthma model was used to observe the effect on relieving asthma. Isolated trachea model was used to observe the effect on relieving spasm. Compared with the control group, the low, medium and high doses of aqueous extracts ofM. Cortex can obviously decrease the frequency of cough, increase phenol red output of trachea in mice, prolong the latent period of asthma in guinea pigs, and increase the antispasmodic rate. The medium dose had the best effect. The comparison between different chemical split fractions ofM. Cortex and the control group showed that the 30% and 50% ethanol fraction ofM. Cortex can obviously decrease the frequency of cough and prolong the latent period of cough induced by ammonia water in mice, increase phenol red output of trachea in mice (P<0.05 orP<0.01); and 30% ethanol fraction and fatty oil fraction can prolong the latent period of asthma in guinea pigs (P< 0.05 orP<0.01). In addition, 30% ethanol fraction can obviously reduce the degree of tracheal contraction. It had certain effect of relieving spasm. It was concluded that aqueous extracts ofM. Cortex had better effects on relieving cough, expectorant and antiasthma. The effective part was the 30% ethanol fraction, which was the dose equivalent of 1/3 of the medium dose. It had significant effect on relieving cough, expectorant and antiasthma. The effect was equivalent to the medium dose of aqueous extracts of M. Cortex.

ABSTRACT

The toxic and teratogenic effect of AlCl_3 on leghorn chick embryos were studied. Different concentrations of AlCl_3 dissolved in redistilled water were injected into the air sac of the eggs (0.2 ml per egg) on the third day of incubation. It was identified that the maximal permissible dosage was 1.708 ?mol/egg, the minimal lethal dosage was 2.25 ?mol/egg, the absolute lethal dose was 35.34 ?mol/egg, and LD_(50) was 8.09 ?mol/egg. The mortality of the embryos raised with the increase of the dose of AlCl_3. AlCl_3 also retarded the development of chick embryos and showed apparent tetratogenic effects.
